EEG in NCSE

چکیده

onconvulsive status epilepticus (NCSE) is defined as prolonged or recurrent electrographic seizure activity lasting more than 30 minutes without prominent motor (i.e. convulsive) clinical symptoms. There are as many types of NCSE as there are nonconvulsive epileptic seizures (Table 1). The hallmark of NCSE is an altered level of consciousness, which may range from mild confusion, inattention, or bizarre behavior to stupor or coma. Motor movements such as automatisms, eye deviation or nystagmoid eye jerks, or limb or facial twitching may be seen. Because NCSE cannot be clinically distinguished from other conditions causing stupor or coma, the electroencephalogram (EEG) is an essential tool for the diagnosis and classification of NCSE. The EEG criteria for NCSE are subjective and somewhat arbitrary, however, and even experienced electroencephalographers often disagree on whether a particular EEG pattern represents NCSE. This article will review the EEG diagnostic criteria for NCSE and common EEG patterns that can be confused with NCSE.

Nonconvulsive status epilepticus and coma.

Nonconvulsive status epilepticus (NCSE) in a comatose patient cannot be diagnosed without electroencephalography (EEG). In many advanced coma stages, the EEG exhibits continuous or periodic EEG abnormalities, but their causal role in coma remains unclear in many cases. To date there is no consensus on whether to treat NCSE in a comatose patient in order to improve the outcome or to retract from...

Non-convulsive status epilepticus: usefulness of clinical features in selecting patients for urgent EEG.

BACKGROUND Non-convulsive status epilepticus (NCSE) is status epilepticus without obvious tonic-clonic activity. Patients with NCSE have altered mental state. An EEG is needed to confirm the diagnosis, but obtaining an EEG on every patient with altered mental state is not practical. OBJECTIVE To determine whether clinical features could be used to predict which patients were more likely to be...

Clinical presentation, epidemiology, neurophysiological findings, treatment and outcome of nonconvulsive status epilepticus: a 3-year prospective, hospital-based study

Objective: This study reports the prevalence of Nonconvulsive Status Epilepticus (NCSE) in patients with altered mental status (AMS), and describes the clinical presentation, etiology, neurophysiological findings, neuroimaging, treatment, and outcome of NCSE in Qatar. Recording duration of continuous EEG monitoring was also discussed. Methods: This was a 3-year, prospective, hospital-based stud...

Nonconvulsive Status Epilepticus in the Neurosurgical Setting

Herein, we review the current state of nonconvulsive status epilepticus (NCSE). NCSE has recently been recognized as one of the causes of unexplained impaired consciousness in the neurosurgical or neurocritical setting. The causes of NCSE include not only central nervous system disorders such as craniotomy, stroke, traumatic brain injury, and central nervous system inflammation, but also severe...

Emergency ("stat") EEG in the era of nonconvulsive status epilepticus.

The most widely accepted indication for a stat EEG (stEEG) is the suspicion of nonconvulsive status epilepticus (NCSE). NCSE has been reported with surprising frequency in a wide variety of acute structural and metabolic brain injuries and significantly increases the risk of permanent brain damage and death. This risk rises and the effectiveness of treatment decreases with delays in diagnosis a...
